Welcome to FitAF Performance.
Before your first session, we require all clients to complete a short onboarding process. This ensures your program is built safely, efficiently, and tailored specifically to your goals, movement patterns, and current condition.
Our system is structured, data-driven, and designed to deliver high-level results while minimizing risk.
Step 1 — Complete Your Client Intake (ONLY FOR ONLINE COACHING/IN-PERSON COACHING)
This form allows us to understand:
- Your training experience
- Current goals
- Injury history and limitations
- Lifestyle and recovery factors
Step 2 — Sign Liability Waiver & Consent (FOR MANUAL THERAPY & ALL OTHER SERVICES)
All clients must review and sign our waiver prior to participating in any services.
This document covers:
- Assumption of risk
- Manual therapy consent
- Training and performance services
- Liability release
Step 3 — Schedule Your Session
Once your intake and waiver are complete, you can schedule your session. (SKIP IF ALREADY BOOKED)
Important Notes
- All forms must be completed prior to your session
- Failure to complete onboarding may result in cancellation or rescheduling
- Please disclose all injuries and medical conditions honestly
